2024 GYF SEASON INFO
UPDATE: OSAA PHYSICAL FORM IS NOW REQUIRED FOR ALL GYF ATHLETES (flag, tackle & Cheer)
All football players and cheerleaders must have a completed OSAA Physical Examination Form physically signed by a doctor or nurse practitioner. NO stamp signatures. This is the only form we can accept for TVYFL.
- The physical exam date will need to cover for the entire football season period
- Physical exams are valid for two years from the exam date
For medical form questions or to check if your player's form is valid from last season, please email our Player Safety Director, Vanessa Villanueva
All players must turn this form in before they can participate or receive their gear.
